The Death Screen is what the player sees when dying by an entity on a run. It can be disabled in the Settings.
Description[]
When a player dies, their screen will quickly fade into red static that eventually turns into white static, consuming their vision. Their screen will then "turn off" much like how an old television would. Eventually, the player's screen will proceed into the Death Screen. The screen won't static if the player was killed by A-666.
Once the player is in the Death Screen, they will see a CRT TV with a tape on top of it. This screen a song unique to the game called "Death Tapes" by Alica. If the player died from an entity, the tape on top of the television would have its label changed to the entity's name. A video on the television and a journal will also be shown to the player, the video gives an example of how to avoid, the journal gives some topics, some of them, teaching you how to avoid next time.
The entity shown is not necessarily the last entity to damage/kill the player but rather the entity most responsible for the player's death. For example, if the player dies to A-60 due to Shell, the latter is the one displayed on the death tape. This is done to give more relevant tips (in the example given, the player would likely already be familiar with A-60's mechanics but may not yet know how to avoid Shell).
If the player died to no entity, they won't see the television, tape, or journal. They will just see a black void. Resetting your character or falling in the pit of blood will cause this.
The Death Screen has one button on the top left, labeled as "Lobby", that transports the player back to the main menu. In multiplayer, an extra button is displayed that will respawn the player as a ghost when clicked, granted that at least 1 or more players are alive (the spectate button will not appear in multiplayer if all players are dead).

Trivia[]
- There used to be a bug where entity names on the Death Screen's tape could bug out.
- There's a possibility of hearing Entities from the death screen, however it's very rare to occur in a run.
- It is possible that an upcoming update (that may add A-300 into Rooms & Doors) will give the Death Screen a new "Revive" button.
- CJ4 has since confirmed it.
- In older versions of Rooms & Doors, if the player didn't die to an entity, there would still be a TV, but its tape would say "Rooms & Doors," and the screen would be pure static. There would also be no journal.
- If you died/Encountered an entity, the game will show a part of the drawing at the main menu, also the memories will show that entity and you can click to "remember" it.
- The Death Screen changes depending on whether you have reached A-1000 or not.
- Whenever you reach A-1000, the death screen will be different with a new death soundtrack, different colored notebook and the TV edges being brown instead of black. This only occurs when you die ONLY if you have been to A-1000 at least once.
- The Death Screen seen is NOT Your sight. Rather, The Guest's, which you find at Room 0.
- The "Death Tapes" song somewhat resembles the "Curious Light" song from Doors.
- In the 7M Update, players can now view the black and white death screen, even if they have reached A-1000. This can be seen by toggling off the "Omega Death Screen" in the settings tab in main menu.
- Dying to A-250 because of the bell in the power room will skip the death screen.
- A-10's hole, A-666, Red Skull and THE VOICES are the only entities who doesn't have a death screen. Instead, they will simply skip to the lobby and spectate buttons, or send players to the Serious Room in the case of Badskull.
